SMS Terms of Service

By opting in to receive SMS (text) messages from Medical Advancement Centers of Arizona (MACoA), you agree to these SMS Terms of Service. Please read them carefully.
1. Program Description
MACoA uses SMS to provide updates about your appointments, general health-related information, promotional offers, reminders, and other service-related communications.
2. Message Frequency
Message frequency may vary depending on your interaction with MACoA services. You may receive periodic messages per month including appointment reminders, confirmations, follow-up messages, and occasional marketing content.
3. Opt-In
By providing your mobile number and checking any box labeled to give consent or sending a keyword to a designated number (e.g., “JOIN”), you consent to receive recurring messages from MACoA at the number provided. Consent is not a condition of purchase or receiving services.
4. Opt-Out
You can cancel SMS messages at any time by replying with any of the following keywords:
STOP, END, CANCEL, UNSUBSCRIBE, or QUIT.
After opting out, you will receive a one-time confirmation message. No further messages will be sent unless you re-enroll.

6. Message and Data Rates
Standard message and data rates may apply from your mobile carrier. MACoA is not responsible for any charges incurred from text messaging including charges from mobile carriers or third parties.
7. Carriers
MACoA SMS messages may be delivered via major mobile carriers. However, MACoA and mobile carriers are not liable for delayed or undelivered messages.
8. Privacy
Your privacy is important to us. All data collected through our SMS program is governed by our Privacy Policy. We do not share your mobile number or personal data with third parties for marketing purposes without your consent.
9. Eligibility
You must be at least 18 years old or have parental/guardian consent to participate in MACoA’s SMS services.
10. Modifications
MACoA may update or modify these SMS Terms of Service at any time. Updates will be posted to this page with a new effective date. Continued use of SMS services following any changes indicates your acceptance of those changes.
